Process for Requesting Outreach Services

Download the appropriate Outreach Request for Services form(s). Requests may be for general resource information, home consultation, local on-site school/ECI program consultation and/or training.

New! Theory to Practice Consultation visits - unlike the School Consultation, this model provides local teacher training on requested topics rather than a focus on a specific student's program. District and Outreach staff collaborate to custom design the consultation. It may include face-to-face instruction, distance learning, ongoing contact and other forms of support. The goal is to help educational teams implement evidence-based practices for students with visual impairments or deafblindness. Requests will be routed to the appropriate Outreach consultant, who will get in touch with you promptly. The requests for school consultation and/or training will be shared with an Outreach consultant who will set up a time and date for an in-depth phone call to the designated contact person on the Outreach Request for Services form. We will also share the request with the local Regional Education Service Center, as appropriate, to see if their resources can help address your questions.
The Outreach consultant will discuss next steps that might be helpful before or instead of an on-site school visit, such as:
- strategies to try,
- resources to access,
- additional ideas for assessment,
- checklists and forms
