APH: Federal Quota Ordering

TSBVI manages a statewide system of ordering, distributing, and repairing instructional materials purchased from APH with federal quota funds. Specialized equipment and instructional materials may be requested by a local school district for the eligible visually impaired students it serves. Visit APH Products.

Ordering APH materials with Quota Funds

Use the forms below to place orders for APH materials. Open the form and please fill it out completely, including your school district's county-district number, the shipping address, and the contact information on the bottom of the page. Then save the form and send it as an attachment to txaph@tsbvi.edu

You will receive an email confirmation of the order when we process it.

When returning items, either to re-stock in the warehouse or for repair, please use the return form below. You do not need to email a copy of the return form but please do include the form with the item. Send return items to:
